RQA-based EEG Analysis for Assessing Motor Hand Recovery in Children with Hemiparetic CP After Interactive Rehabilitation Protocol: A Case Study

چکیده :
Hemiparetic Cerebral Palsy (HCP) is a neurodevelopmental disorder caused by unilateral brain damage that impairs motor function, particularly in the upper limbs. While game-based rehabilitation effectively encourages engagement, clinical tools often fail to capture brain-level dynamic changes. This pilot case study investigated the impact of a novel interactive game-based rehabilitation protocol on hand motor recovery in five children with HCP. EEG signals were recorded before and after a 12-session intervention. Recurrence Quantification Analysis (RQA) was used to extract four robust features—Recurrence Rate (RR), Determinism (DET), Entropy (ENTR), and Average Diagonal Line Length (L)—from the Fz, C3, and C4 channels. Results showed significant increases in Fugl-Meyer Assessment (FMA) scores across all subjects, indicating clinically significant improvement. In parallel, RQA measures in the motor cortex (C3 and C4) revealed distinct dynamic shifts: while RR showed a slight increase, a significant decrease was observed in DET, ENTR, and L. This reduction in DET and L signifies a transition from pathologically rigid and repetitive neural activity to more flexible, complex, and adaptive dynamical states. Furthermore, the decrease in ENTR suggests a more organized and efficient information flow with reduced stochastic noise. The correlation between reduced deterministic rigidity and higher FMA scores proves that this protocol effectively enhances motor capabilities by fostering neuroplasticity and breaking stereotypical neural firing patterns.
