An Interactive Intelligent Gaming System for Movement Rehabilitation in Hemiparetic CP Children: A Case Study

چکیده :
Spastic Hemiparetic cerebral palsy (HCP), characterized by increased muscle tone and impaired motor coordination, remains a significant challenge in pediatric rehabilitation. This study presents the design and quantitative evaluation of an integrated intelligent gaming system developed to enhance motor recovery through adaptive cognitive-motor training. The proposed setup synchronizes a stationary bicycle with customized joysticks to facilitate simultaneous bimanual and lower-limb interaction within a competitive virtual environment. To provide an objective assessment beyond subjective clinical scales, a comprehensive framework integrating Electromyography (EMG) and Electroencephalography (EEG) was implemented. Muscle synergy was quantified using the Hierarchical Alternating Least Squares (HALS) algorithm, while directed cortical connectivity was evaluated via non-linear measures, including Granger Causality (GC) and Transfer Entropy (TE). Results from a 10-week intervention on two HCP patients demonstrated notable improvements in Fugl-Meyer Assessment (FMA) scores, particularly in upperextremity motor function. Neurophysiological analysis revealed a systematic reorganization of muscle synergy matrices (W and C) and a significant modulation of neural information flow within motor networks. These findings suggest that the proposed rehabilitation protocol offers a robust, technology-based approach for HCP rehabilitation, providing quantifiable markers of neuroplasticity and motor recovery.
