Does Lower Extremity Rehabilitation Affect The Recurrence Dynamics of Hand EMG Signals? A Case Study

چکیده :
Spastic Hemiparetic Cerebral Palsy (HCP) presents significant motor control disorders, particularly in distal joints like the ankle. This study examines the efficacy of a novel interactive intelligent gaming system designed for adaptive cognitive-motor training in driving motor recovery for two children with HCP. The intervention's clinical effectiveness was confirmed by an improvements in lower-limb Fugl-Meyer Assessment (FMA) scores, particularly in ankle control capabilities. To provide objective and quantitative validation beyond subjective clinical scales, Electromyography (EMG) signals were recorded pre- and post-intervention from the affected limb’s Tibialis Anterior and Gastrocnemius muscles. Recurrence Quantification Analysis (RQA) was employed as a robust nonlinear method to quantify muscle dynamics using features including Recurrence Rate (RR), Average Diagonal Line Length (L), and Recurrence Entropy (ENTR). Results revealed systematic changes corresponding to the clinical gains. In the Tibialis Anterior, the mean L and ENTR increased (L: 2.98 to 3.48; ENTR: 0.89 to 1.12). This pattern, alongside a slight decrease in RR (0.0865 to 0.0747), indicates a shift toward more stable, adaptive, and efficient voluntary control. Conversely, the antagonistic Gastrocnemius showed a uniform decrease across all features (L: 34.74 to 33.09; ENTR: 4.12 to 3.49; RR: 0.8060 to 0.7765), confirming reduced detrimental co-contraction and non-functional activity. We conclude that the interactive intelligent gaming system is an effective intervention tool that promotes positive neuro-muscular reorganization in the ankle. RQA of EMG signals is established as a powerful, quantitative method to objectively monitor and explain the physiological mechanisms of recovery following such interventions.
